Please fill out the form below to nominate a veteran or active duty service member to receive a wish from Soldier’s Wish. Make sure to include a detailed description of the wish and why you are making the nomination.

The “Wish Nomination Form” is the first step in the nomination process. It is not a confirmation of eligibility for a Soldier’s Wish. Your information will be sent to our Wish Committee for review.
